Understanding Support at Home

Support at Home is Australia’s reformed aged care program, effective from 1 November 2025. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme.
The program provides government-funded, coordinated care and services to help older Australians remain living independently at home. It’s designed to be more flexible, person-centred, and responsive to individual needs.
The Commonwealth Home Support Program (CHSP) will transition to Support at Home no earlier than 1 July 2027.

What Support at Home covers

Your Support at Home funding can be used to access a wide range of in-home services. Fambulcare helps you connect with the right registered providers to deliver these supports.

Personal Care

Assistance with bathing, grooming, dressing, and personal hygiene — delivered with dignity and sensitivity.

Domestic Assistance

Help with cleaning, laundry, vacuuming, and keeping your home safe and comfortable.

Meal Preparation

Planning and preparing nutritious meals that suit your dietary needs and cultural preferences.

Shopping & Errands

Grocery shopping, pharmacy runs, and other essential errands to keep your household running smoothly.

Transport

Safe and reliable transport to medical appointments, community activities, and social outings.

Social Support

Companionship, accompanying you to activities, and helping you stay connected to your community.

Medication Support

Reminders and assistance to ensure you take the right medications at the right time.

Home Maintenance

Minor home repairs, garden maintenance, and safety modifications to help you live independently.

How to access Support at Home

Contact My Aged Care

Call My Aged Care on 1800 200 422 or visit myagedcare.gov.au to register and request an aged care assessment.

Undergo a Single Assessment

An assessor will visit you at home to determine your care needs. The new Single Assessment System (from 1 July 2024) simplifies this process.

Receive Your Approval

Once approved, you’ll receive confirmation of your eligibility for Support at Home and be placed on the national queue.

Support at Home is the new Australian Government-funded aged care program that replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. It provides coordinated care and services to help older Australians remain living independently at home.
The Single Assessment System (effective 1 July 2024) is a simplified aged care assessment process. Instead of multiple assessments, you now have one comprehensive assessment that determines your eligibility for aged care support, making the process faster and easier.
You may be eligible if you are aged 65 or over (or 50 or over for Aboriginal and Torres Strait Islander people), and have care needs that cannot be fully met by family, friends, or community services. A Single Assessment will determine your eligibility.
The Australian Government subsidises the majority of the cost. You may be asked to contribute a basic daily fee and, depending on your income, an income-tested care fee. A fee assessment through Services Australia will determine your contribution.
Wait times vary depending on your assessed care needs and your position on the national queue. Fambulcare can help you navigate the process and access interim support while you wait.
Yes. You have the right to change your Support at Home provider at any time. Your unspent funding will be transferred to your new provider. Fambulcare can guide you through the transition process.
Fambulcare is currently working towards full registration as an aged care provider. In the meantime, we support clients to navigate the aged care system and broker Support at Home services through registered providers on your behalf — ensuring you receive high-quality, person-centred care at home.
The Home Care Packages Program was replaced by Support at Home on 1 November 2025. If you were receiving HCP services, your care has transitioned to the new Support at Home program automatically. The Commonwealth Home Support Program (CHSP) will transition to Support at Home no earlier than 1 July 2027.
